We strongly encourage the submittal of questions to be added to this list. We also encourage you to submit a solution

This form is to be used by customer’s wishing to express their complaints/concerns pertaining to MSI Products. This
information shall be forwarded to the Quality Manager to process in accordance with Dixie Iron Works, Ltd. Operation Procedure 19-02, Field
Non-Conformance Report. The Quality Manager shall determine whether the complaint/concern is either Significant and/or Non-significant.
Complaints deemed Significant shall be processed in accordance with Operation Procedure 21-02, Corrective/Preventative Action Report. Once the
report is completed a copy shall be forwarded to the customer. Those complaints initially deemed Non-Significant will be followed up with customer contact for further investigation.
